Course Introduction
This course is based on the theory of Iyengar, very suitable for beginners. During the practice we use yoga bricks and yoga ropes to help students find yoga secquence movement in correct way under safety and stable body posture and alignment. Practitioners can learn about basic anatomy and better understand their body's correct movement patterns, safely entering more difficult movements, avoiding injuries and relieving physical and mental stress.

❤️ 胸鎖乳突肌:從耳後顳骨的乳突部,沿著頸部下滑,撫摸你的胸鎖乳突肌,直到鎖骨頭的位置。短短十幾公分,是我轉頭望向你的距離。
❤️ 心尖搏動觸診區:輕輕地把你的手放在這。你知道摸這個位置,最能感受到我強烈的心跳嗎?
❤️ McBurney’s Point:你隨時可以從背後摟上我,緊緊環繞我的腰腹,在耳邊對我呢喃,直到我闌尾發炎的那天。
❤️ 肱三頭肌和肱二頭肌:只有在你的三頭肌和二頭肌之間選擇時,我才曾經對你三心二意。
❤️ 肱骨的尺神經溝:今晚你可以抽打我身上的每個部位,讓我臣服於你。但這裡不要。因為會麻。(對不起好像口味太重orz… )

■ 解剖說明
○ 胸鎖乳突肌 Sternocleidomastoid muscle:轉頭時會用到的肌肉。 Muscle that rotates your head laterally.
○ 心尖搏動觸診區 Plpation area for apex beat:正常位於第五肋間與鎖骨中線交界處。但若有心臟肥大等異常狀況,則可能偏離該處。It is located at 5th intercostal space within midclavicular line in normal individuals. Might be displace if there conditions such as cardiomegaly.
○ McBurney’s Point:肚臍與右側前上髂棘連線,下三分之一處。為闌尾炎患者肚子常出現壓痛的部位。It is located at one-third of the distance from the anterior superior iliac spine to the umbilicus.
○ 肱三頭肌和肱二頭肌 Biceps brachii muscle and triceps brachii muscle:使關節彎曲與伸直。Flex and extend the elbow joint.
○ 肱骨的尺神經溝 Groove for ulnar nerve:位在肱骨遠端靠內側處。尺神經通過此處,因此不小心撞到時,常常會覺得手掌小指側又痛又麻。 Located at the medial side of the distal humerus. Accidentally bumping that location would cause numbness and pain over the medial side of the palm, since ulnar nerve is impacted.

在中國文字上,一字之差,往往可以表達兩種截然不同的意思。同一件事情,「何德之衰(《論語.微子》)」「何如德之衰(《莊子.人間世》)」便可以衍生出儒道兩派不同的意思。
在藥物上,這種情況,同樣出現。
在藥理上,Hyoscine,是一種毒蕈鹼受體(Muscarinic receptor)拮抗劑,可以透過跟乙醯膽鹼(Acetylcholine)進行競爭,抑制乙醯膽鹼的作用,從而達到抗毒蕈(Antimuscarinic)的效果。
Hyoscine butylbromide,跟Hyoscine hydrobromide最大的不同,在它們不同的化學結構:
在結構上,Hyoscine butylbromide,是一種季銨化合物(Quaternary ammonium compound):一個氮原子(Nitrogen)連著四個碳原子(Carbon)(-NR3+);Hyoscine hydrobromide,是一種叔胺(Tertiary amine):同一個氮原子,卻連著三個碳原子和一個氫原子(Hydrogen)(-NR2)。
插播一下,從化學名稱的中文譯名,多多少少,也能看出一些中國文化的底蘊:
在中國文字上,兄弟長幼,排行次序,伯是最大,仲是第二,叔是第三,季是最小。(以時長幼,號曰伯、仲、叔、季也。伯者,子最長,迫近父也。仲者,中也。叔者,少也。季者,幼也。《白虎通.姓名》)
在中國歷史上,漢高祖劉邦,有時候,稱為劉季,在相當程度上,便是這個原因。
所以,胺(Amine)的氨原子,根據氫原子被取代的數量,按照順序,可以分為伯胺(Primary amine)、仲胺(Secondary amine)、叔胺、季銨鹽。
看到這裡,看倌可能會問:
「在結構上,兩者這一點分別,有什麼關係呢?」
在結構上,這點分別,已經足以導致藥理上的差異。
念過化學的看倌,可能會知道,季銨化合物,因為一個氮原子連著四個碳原子,所以,它便成為了一種帶正電荷的離子,稱為「陽離子(Cation)」,因為帶正電荷,所以,會大大減少藥物在腸胃的吸收,同時,因為帶正電荷,會增加藥物的親水性,減少藥物的親脂性,從而減少血腦障壁(Blood-brain Barrier)的穿透性,所以,較難進入大腦,影響中樞神經系統,產生抗膽鹼(Antichlorinergic)的藥效。
簡單說,Hyoscine butylbromide,作為一種季銨化合物,主要直接在腸道內發揮藥效,用作一種解痙劑(Antispasmodic),較適用於治療腸易激綜合症(Irritable Bowel Syndrome, IBS),透過放鬆腸道的平滑肌(Smooth muscle),紓緩肚痛(Abdominal pain)、腸抽搐(Intestinal spasm)的症狀。
值得一提,因為Hyoscine butylbromide可以放鬆腸道的平滑肌,所以,同時可能會放鬆連接食道、胃部的食道下端括约肌(Lower oesophageal sphincter, LOS),從而不能緊緊關閉胃部的入口,惡化胃液倒流(Gastroesophageal reflux disease, GERD)的症狀。如果用藥者罹患胃液倒流的話,建議謹慎用藥。
相較Hyoscine butylbromide而言,Hyoscine hydrobromide,作為一種叔胺,不帶電荷,所以,擁有較大的親脂性,較能穿透血腦障壁,直達大腦,較適用於預防、治療動暈症(Motion sickness),預防、紓緩眩暈、噁心、嘔吐的症狀。
在副作用上,相較季銨化合物而言,叔胺擁有較大的親脂性,所以,Hyoscine hydrobromide較能影響中樞神經系統,從而較常產生嗜睡、口乾、視力模糊、小便困難、便秘這些副作用。
