1) if you have slot allocated for you ELSEWHERE, we won't be putting you on the list here.
2) although we prioritise your details to MOH it doesn't guarantee that you may be allocated to our clinic for vaccination.
3) The time and date of vaccination will be DETERMINED by MOH itself.
4) we DO NOT know what TYPE of vaccine will be provided and the brands of vaccine may change from time to time
5) It is FOC and the program is under MOH so we will not charge you for this particular service.
