<產前胎兒貧血之診斷>

大人會貧血，寶寶也會貧血。最近遇到相關case，來整理一下。

#胎兒貧血的原因:

1. 溶血: 最多是血型不合的溶血，在產前以Rh血型較常見，另外比較少見的是Kell、Duffy和Lewis。

2. 感染: Parvovirus B19會抑制紅血球生成，因此也會造成貧血。如果可以即時進行胎兒輸血來校正，通常撐過去後預後都不錯。

3. 胎兒母體間出血 (Fetomaternal hemorrhage)

4. 雙胞胎輸血症候群/twin anemia polycythemia sequence

#產前要怎麼確定胎兒貧血的狀況?

1. #超音波!!! (我們最常使用 #非侵入性的工具)
目前公認的就是16週以後量測 #中大腦動脈的血流流速 (MCA PSV)。
我們會比較MCA PSV的MOM值(課本或網路都可以查到週數對照的值)，MOM>1.25會要開始懷疑有貧血的狀況，如果MOM>1.5則有可能需要進行胎兒輸血了。

2. 臍帶血
3. KB test
另外抽血也可檢驗血型的抗體濃度。當抗體過高超過一定濃度，搭配MCA血流出現變化，會要更密集的追蹤! (大約1-2週就要追蹤中大腦動脈的血流)

#產前可以做甚麼
胎兒治療的部分就是進行胎兒輸血。
1. 臍靜脈: 和臍帶血穿刺的方式一樣，以羊膜穿刺的針目標對準臍靜脈。
2. 胎兒腹內的肝門靜脈
3. 腹腔內輸血: 適合週數小，血管較小的案例。
