Crazy week photo dump. It feels like this week was made up of 20 days instead of 7. I can barely believe it went by so fast and yet so oddly slow at the same time. Looking through my camera roll.. made me realise that my current lifestyle ain’t helping my soaring cholesterol and health problems that are waiting to arise. It’s worrisome but I keep telling myself “Not this week. Not this week” and then feeling disappointed all over again at the next blood test results.

Give me the strength and discipline to say “no” & consciousness to prioritise myself once September ends 💪🏻 PS. I know nobody read captions anymore which is why I’m feeling less stressed about making it sound coherent here - just a mental note to myself and putting it out there❤️

COVID-19 的兒科急診日記

今天國內新增334例本土個案，另外校正回歸256例。

今天再新增6例死亡，累計29例死亡。

大部分的死亡病人，很多都是中高齡，而且都還是有慢性疾病。

今天重要的新聞：

1.中央流行疫情指揮中心評估雙北需要跨區支援，未來可能持續增加重症，因此請各大醫院依照規模準備10到20個加護病房，提供治療相關患者。

2.雙北快篩陽性高達9成左右確診，但雙北以外縣市快篩，偽陽性普遍。

這兩天大家都在談論「隱形缺氧」。

我來稍作補充說明

「隱形缺氧」

“Silent hypoxia" is terrible ‼️

那甚麼是隱形缺氧？

定義: 病人的血氧飽和度水平低於預期的飽和度（病人的血氧飽和度是〜50–80％，而預期的血氧飽和度為95％或更高），但是該病人沒有任何呼吸困難。

這種“隱形缺氧”的出現，真的令醫師們感到擔憂！

Why ?

原因在於這種缺氧最初不會引起呼吸短促，但可能會在患者內部造成快速損害。

這類病患的臨床表現:

患者通常會在就醫前2到7天出現一些症狀，如突然胸悶或無法深呼吸，幾天後，呼吸非常急促，幾乎是正常呼吸速率的三倍。

病患的嘴唇和手指也有點發青。

直到醫師檢查了血氧濃度，才真正明白病患的病情有多嚴重。

根據美國醫生們2020年實戰經驗分享，一些到急診室就診的COVID-19患者血氧飽和度已下降到70至80%之間，有些病人甚至低於50%，他們卻仍能正常行動。

「那如何早期檢測隱形缺氧」

我們若是能早期檢測隱形缺氧，是可以有助於決定何時開始氧氣治療或是判斷用侵入性或非侵入性的給氧方法。

有3種方法可以找出隱形缺氧的存在。

1. 脈搏式血氧儀(Pulse oximeter): 用來監測病人的血氧飽和度(SpO 2)（圖）

在醫療人員的指導下可以使用的。

透過脈搏式血氧儀不斷監測SpO 2是檢測隱形缺氧的一個很好的選擇。

正常的SpO 2應維持在94%以上。

但我們是要確保使用的脈搏式血氧儀具有較高的準確率，尤其是在患者SpO 2低於90％的情況下。

2.動脈血液氣體分析(Arterial blood gas analysis) (ABG analysis)

這需要在醫療單位使用。

動脈血液氣體分析是從動脈中採集血液樣本以檢查血液中不同氣體濃度的過程。

ABG和SpO 2可能是早期檢測COVID-19患者隱形缺氧的有效的方法。

3.6分鐘步行測試（six-minute walk test）(6MWT)

在醫療人員的指導下可以使用的。

自己在步行6分鐘後測量SpO 2

這個方法，如果病人有脈搏式血氧儀(Pulse oximeter)，是可以請無症狀或輕症病人在家自我檢測。

患者以正常的速度步行六分鐘，以監控其生理反應。

已被證明的是，在執行6MWT後，COVID-19的患者更容易患上運動性缺氧無自覺症狀。

因爲這類COVID-19患者會出現運動引起的缺氧而沒有呼吸困難。

與對照組相比，COVID-19患者的SpO 2水平顯著降低。

一般來說，在步行6分鐘內，SpO 2低於90％應該定義是異常的。

此外，SpO 2下降超過4％的患者也可能被認為是嚴重病例。

但這還是要病人能夠正確使用儀器。

最後，我們絕對不要低估「隱形缺氧」

What if I can’t see the supply?

“All the congregation of the children of Israel traveled from the wilderness of Sin, by their journeys, according to Yahweh’s commandment, and encamped in Rephidim; but there was no water for the people to drink. Therefore the people quarreled with Moses, and said, “Give us water to drink.” Moses said to them, “Why do you quarrel with me? Why do you test Yahweh?” The people were thirsty for water there; and the people murmured against Moses, and said, “Why have you brought us up out of Egypt, to kill us, our children, and our livestock with thirst?” Moses cried to Yahweh, saying, “What shall I do with these people? They are almost ready to stone me.” Yahweh said to Moses, “Walk on before the people, and take the elders of Israel with you, and take the rod in your hand with which you struck the Nile, and go. Behold, I will stand before you there on the rock in Horeb. You shall strike the rock, and water will come out of it, that the people may drink.” Moses did so in the sight of the elders of Israel.”(Exodus‬ ‭17:1-6‬ ‭WEB‬‬)

The name of the place “Rephidim” in the wilderness of Sin means “place in the desert”. When the children of Israel were encamped at Rephidim, they were thirsty and there was no water source in sight.

This represents a situation in life where you can’t see the supply. It’s like you feel trapped with no possible way out.

When we walk by sight, we will be more conscious of the lack.

However, when you trust God, you will know that even in an ‘impossible’ situation, God has already prepared the supply that you need.

God will bring the water of supply out of the rock for you.

“and all drank the same spiritual drink. For they drank of a spiritual rock that followed them, and the rock was Christ.” (1 Corinthians‬ ‭10:4‬ ‭WEB‬‬)

The apostle Paul taught us the revelation that the rock was a picture of Christ.

God brought nourishment and life out for us when Christ was stricken. When did this happen?

At the cross, Jesus was punished for our sins so that we could receive His righteousness, and obtain every blessing from God.

He bled rivers of blood so that He could make rivers of living water flow inside us.

In whatever desert situation you find yourself in now, keep your eyes on Jesus. The water you need to quench your thirst will come from Christ who was stricken for your sake!

Forest bathing otherwise known as shinrin-yoku has shown to decrease stress, increase creativity, increase immunity and lower blood pressure. However it sounds a bit too good to be true so that is why I headed out to the beautiful forests in Guia Park, Macau to test it out.

Join me on my cinematic hiking adventure through the vibrant forests of Macau, China. As I test out the nature therapy called "Forest Bathing".
