四時四十五分, 已接近老人科門診的尾聲。今天負責的是 Drug Compliance Clinic <<藥物依從門診>>, 老友記會先見藥劑師, 才見醫生。
一位80歲的婆婆推門進來。翻查紀錄, 她中過風, 又有血壓高、糖尿病、心房顫動等常見的長者問題。她被轉介來 Drug Compliance Clinic, 是商討使用新藥物的問題。
話說心房顫動在老友記當中非常普遍, 而患有心房顫動可導致中風。為了預防中風, 我們會為老友記處方薄血藥。吃舊式薄血藥要戒口、又要定期抽血檢查血凝指數, 確實有點麻煩; 故大概十年前推出的新一代薄血藥可說是一大進步, 因為它既不需要戒口, 亦不需要定期抽血, 使用當然方便得多; 加上政府最近資助了這種較為昂貴的藥物, 只要合符資格便能免費使用, 對醫生和病友而言, 當然是一大喜訊。
而婆婆呢, 只是吃 Aspirin 亞士匹靈, 對有心房顫動的老友記而言, 預防中風的作用着實不大。
「婆婆, 上次覆診到宜家有無唔舒服? 」第一次見她, 我先打開話題。
「無呀, 無乜唔舒服。」婆婆說得淡然。
「心胸痛、氣喘、心跳快呢? 」
「都無呢。」
「婆婆呀, 咁上次我地話同你轉隻新藥喎, 你考慮成點呀? 」我轉入正題。
「唔好轉啦, 食番舊個隻咪算囉。」婆婆咬着嘴唇。
「新藥好啲㗎, 預防中風呢隻藥好啲, 你有心房顫動食呢隻藥好好多㗎。」我嘗試游說。
「唔使啦, 唔好轉啦, 食舊個啲咪好地地囉。」婆婆堅持。
「呢隻新藥真係好啲喎, 副作用又少啲, 又唔使好似人地咁又要戒口又要抽血; 宜家政府資助仲唔使錢㗎。」
我差點兒想告訴她, 以前政府未資助, 有些老友記無錢自購, 無奈地放棄使用這種新藥。
「要咁好做乜喎, 有嘢咪死咗去算囉。。。」婆婆竟然突然這樣說。
「你好似唔係咁開心咁嘅, 婆婆。」
「我係唔開心呀。」婆婆雙眼望着地下。
「可唔可以話俾我知乜事呀? 」
婆婆低着頭。。。當場氣氛突然寂靜起來。。。她的眼角開始滲着淚水。。。
「屋企嘢? 」過了一會兒, 我再問:「仔女唔聽話? 」
「唔係, 我同佢地無野呀, 佢地對我幾好。」
「咁係乜野事? 驚身體唔好, 你宜家幾好喎個情況。」我百思不得其解。
「我。。。唉。。。」婆婆欲言又止: 「我個次係病房俾人綁, 我宜家都好唔開心。。。」
話未說完, 婆婆雙眼淚水如泉水般湧出來。
「幾時入院, 邊次呢? 」我立刻望着電腦努力搜索着。
「四月個次, 次次有事睇吓醫院食吓藥就無事, 係個次啲仔女夾硬話我差要我入院, 係病房又無端端綁住我隻手, 又無水飲, 我好慘呀, 我心諗不如死咗去好過。」一邊說婆婆一邊在哭着。
查查紀錄, 婆婆的而且確四月曾入過醫院。 那次她腹部發炎積膿, 情況頗差, 更曾需入住 ICU 深切治療部。可以想像, 婆婆當時需要接受治療, 所以不能進食; 身患重症可能有點紊亂, 故此被繋上安全衣物, 以防她拔走維生儀器。
但想不到, 這段記憶仍然深深記在婆婆的腦海中, 還為她留下如此嚴重的陰影。。。
還許, 縱使在紊亂中, 我們還是保留着一分清醒。
這是一個很好的反省: 在病房內要盡量避免使用約束用物; 就算迫不得已需要使用, 都要不時評估, 盡快鬆綁, 以減少約束衣物的影響。

Most importantly, being in love with your Mr. Right or Mrs. Right is always the greatest power to push us to live our lives to the fullest. She also believes that everyone has the freedom to love and be loved and we are all born equal.
In addition, Vivian recognised that HIV has become a treatable and chronic illness. If patients keep good adherence to the treatment, they can also live life to the fullest.
Our TV advertisement campaign themed “No misconceptions. No discrimination. This is what LOVE is”. The main point of it is to help audiences understand that people living with HIV are still able to have intimate relationships with their partners as long as their own "Undetectable = Untransmittable" level can be reached with HAART treatment and good drug compliance. Join hands with us to achieve zero AIDS discrimination!
