วัคซีนช่วยระงับการระบาดได้แค่ไหน?

วันนี้เป็นอีกวันหนึ่ง ที่จำนวนผู้ติดเชื้อใหม่ของไทยเราเพิ่มเกินหมื่น แต่ในขณะเดียวกัน อีกหลายๆ ประเทศก็เริ่มพุ่งขึ้นสูงเช่นเดียวกัน เช่น อังกฤษที่กำลังเตรียมจะล๊อคดาวน์อีกรอบ หลังจากจำนวนเคสใหม่เพิ่มห้าหมื่นติดกันหลายวัน ฝรั่งเศสที่ขึ้นมาหมื่นสอง

แต่ทั้งที่อังกฤษและฝรั่งเศสนั้นเขามีการฉีดวัคซีนแพร่หลายกว่าเป็นอย่างมาก แล้วเพราะเหตุใดจึงยังมีการระบาดอยู่? ถ้าหากว่าฝรั่งเศสซึ่งใช้วัคซีน mRNA เป็นส่วนมากและสามารถฉีดให้ประชากรได้ถึงกว่า 40% แล้วยังคงมีการระบาดไม่ต่างจากไทย นั่นหมายความว่าวัคซีน mRNA นั้นไม่ได้มีประสิทธิภาพในการป้องกันโรคขนาดนั้นหรือไม่?

คำตอบคือ ไม่จริงเลย และแท้จริงแล้วนี่เป็นหลักฐานที่ชัดเจนว่าเพราะเหตุใดวัคซีน โดยเฉพาะวัคซีนแบบ mRNA จึงเป็นเครื่องมือที่สำคัญที่สุดในการต่อสู้กับการระบาดของโลก

แล้วเพราะเหตุใดทั้งอังกฤษและฝรั่งเศสที่มีจำนวนผู้ฉีดวัคซีนเป็นจำนวนมากแล้ว ยังมีผู้ติดเชื้อต่อวันมากกว่าไทย? ที่เป็นเช่นนี้เพราะเรากำลังเปรียบเทียบจำนวนที่ไม่ได้ normalize เท่ากันสองจำนวน

ลองเปรียบเทียบดูว่า ปี 2020 ในสหรัฐอเมริกามีทารกเกิดใหม่ทั้งสิ้น 3.6 ล้านคน ในขณะที่หมู่บ้านชาวเขาแห่งหนึ่งที่ไม่มีการคุมกำเนิดมีเด็กเกิดเพียงแค่สามสิบคน ถามว่านี่หมายความว่าการคุมกำเนิดในประเทศสหรัฐนั้นล้มเหลวหรือไม่? แน่นอนว่าไม่ เพราะในความเป็นจริงแล้วเราจะต้องคำนึงถึงจำนวนประชากรที่ไม่เท่ากันด้วย (และแม้ว่าตัวเลขประชากรเกิดใหม่ 3.6 ล้านคนจะฟังดูเยอะ แต่แท้จริงแล้วเป็นอัตราการเกิดที่ต่ำที่สุดเป็นประวัติการในรอบ 100 ปี)

เช่นเดียวกัน ในการระบาดของเชื้อไวรัสนั้น แต่ละประเทศนั้นอยู่ในสถานการณ์การระบาดของโรคที่ไม่เท่ากัน ปีที่ผ่านมานั้นประเทศไทยเรามีจำนวนผู้ติดเชื่อค่อนข้างต่ำมาก ในขณะที่สถานการณ์ระบาดหนักที่เรากำลังเจอกันปัจจุบันนี้นั้น ประเทศตะวันตกได้พบกับสถานการณ์นี้ไปตั้งแต่ปีที่แล้ว จึงเท่ากับว่าเรากำลังไล่ตามหลังเขาอยู่ในเรื่องของการระบาด ดังที่เห็นได้จากจำนวนผู้ติดเชื้อรวมของประเทศอังกฤษและฝรั่งเศส ที่อยู่ที่ 5 ล้านกว่าๆ ด้วยกันทั้งคู่ ในขณะที่ของไทยนั้นยังอยู่ที่ประมาณ 4 แสน (ณ วันที่ 19 กรกฎาคม 2021)

และเนื่องจากว่าเชื้อไวรัสนั้นแพร่จากผู้ที่ติดเชื้ออยู่แล้ว จึงเป็นเรื่องปรกติที่ประเทศที่มีจำนวนผู้ติดเชื้อมากกว่า จะมีผู้ติดเชื้อใหม่ที่มากกว่า เช่นเดียวกับที่ไม่ใช่เรื่องแปลกที่ประเทศสหรัฐซึ่งมีประชากรถึง 328 ล้านคน จะมีทารกเกิดใหม่ถึงปีละ 3.6 ล้านคน (ซึ่งก็นับว่าน้อยมากอยู่ดี)

ดังนั้น วิธีหนึ่งที่เราอาจจะเปรียบเทียบการระบาดของโรคได้ ก็คือการนำเอาจำนวนเคสติดเชื้อใหม่ หารต่อจำนวนผู้ติดเชื้อที่มีอยู่ปัจจุบัน ซึ่งหากเราทำเช่นนี้ เราก็จะพบว่า ฝรั่งเศสมีจำนวนผู้ติดเชื้อใหม่ 67 คนต่อผู้ติดเชื้อ 100,000 คน อังกฤษมี 665 คน ในขณะที่ประเทศไทยนั้นมีผู้ติดเชื้อใหม่อยู่ที่...​ 2375 คนในทุกๆ วัน ต่อผู้ติดเชื้อทุกๆ 100,000 คนเท่ากับว่าเรามีอัตราการติดเชื้อใหม่ต่อการติดเชื้อเดิมมากกว่าอังกฤษถึงเกือบ 4 เท่า และมากกว่าฝรั่งเศสถึงกว่า 40 เท่า

ความแตกต่างของการระบาดเกิดขึ้นจากอะไรได้บ้าง? แน่นอนว่ามีหลายปัจจัย อาจจะเป็นเพราะว่าเขา social distancing ได้ดีกว่าเรา? (ไม่น่าจะใช่ เพราะเขาเริ่มที่จะปลดล๊อคดาวน์ และไม่สวมหน้ากากกันแล้ว) การตรวจพบโรคเยอะกว่า มี antigen rapid test ที่แพร่หลายกว่า (ซึ่งควรจะทำให้ผู้ติดเชื้อทั้งหมดเยอะเพิ่มตามไปด้วย) โหลดของระบบสาธารณสุข หรืออื่นๆ

แต่ไม่ว่าเราจะพยายามอธิบายความแตกต่างของตัวเลขที่เกิดขึ้นอย่างไร ก็ไม่น่าจะสามารถอธิบายได้ทั้งหมด สิ่งหนึ่งที่เราแทบจะปฏิเสธไม่ได้เลยก็คือ "การฉีดวัคซีน" เพราะปัจจุบันในไทยมีประชากรเพียง 4.93% ที่ได้รับวัคซีนครบสองโดสแล้ว ในขณะที่อังกฤษและฝรั่งเศสอยู่ที่ 53.6% และ 40.4% ตามลำดับ

ไม่เพียงเท่านี้ หากเราลองนำข้อมูลระหว่างเปอร์เซ็นต์ในการฉีดวัคซีนครบสองเข็มของแต่ละประเทศ เทียบกับอัตราการติดเชื้อใหม่ต่อ ประชากรติดเชื้อ 100,000 คน เราจะได้กราฟดังภาพ

จากกราฟ จะเห็นได้ว่าแม้ข้อมูลจะมีการกระจายตัวพอสมควรตามสถานการณ์ที่แตกต่างกันในโลกของความเป็นจริง แต่โดยรวมแล้วไม่มีประเทศใดที่มีประชากรฉีดวัคซีนเป็นจำนวนมาก ที่มีอัตราการติดเชื้อที่สูงต่อประชากรติดเชื้อเดิม และยิ่งมีอัตราส่วนประชากรที่ฉีดวัคซีนมีเพิ่มมากขึ้นเท่าไร อัตราการแพร่เชื้อต่อของประชากรติดเชื้อก็จะมีแนวโน้มที่จะยิ่งลดลงเพียงเท่านั้น

หากเราเทียบกับประเทศเช่น สหรัฐ ที่เคยมีการระบาดมากที่สุดในโลกเมื่อไม่ถึงหนึ่งปีที่แล้ว ปัจจุบันสหรัฐก็ยังมีประชากรติดเชื้อมากอยู่ถึง 34 ล้านคน (มากกว่าไทยถึง 100 เท่า) แต่ทุกวันนี้ด้วยการระดมฉีดวัคซีนที่มีประสิทธิภาพสูงเช่น mRNA ทำให้สามารถกลับมาใช้ชีวิตปรกติ คนส่วนมากไม่สวมหน้ากาก สามารถจัดคอนเสิร์ต และฉลองวันชาติได้ แต่ก็ยังมีจำนวนคนติดเชื้อเพิ่มขึ้นเพียงแค่วันละ 35,000 คน ซึ่งมากกว่าไทยเพียง 3 เท่า เท่ากับว่ามีอัตราการแพร่เชื้อน้อยกว่าไทย 20-30 เท่าเลยทีเดียว

บางคนอาจจะบอกว่า ก็ยังมีคนอีกเป็นจำนวนมากดที่แม้จะได้รับวัคซีนแล้ว แต่ก็ยังติดเชื้ออยู่ดี และต่อให้ประเทศที่ฉีดวัคซีนเยอะแล้ว ก็ยังพบคนที่วัคซีนครบติดเชื้อเพิ่มขึ้นทุกวันๆ

แต่แท้จริงแล้ว การพบผู้ที่ได้รับวัคซีนครบแต่ยังติดเชื้อได้นั้น เป็นสิ่งที่ควรจะเกิดขึ้นอยู่แล้ว และแท้จริงแล้วเป็นสัญญาณที่ดีของการเข้าสู่การยับยั้งการระบาด ลองจินตนาการดูว่า ประเทศหนึ่งที่คนสูบบุหรี่จัดมาก จนมีผู้เสียชีวิตจากมะเร็งปอดเป็นจำนวนมาก ซึ่งส่วนมากสูบบุหรี่ด้วยกันทั้งนั้น ในขณะที่อีกประเทศหนึ่งนั้นห้ามสูบบุหรี่โดยเด็ดขาด และไม่มีประชากรคนใดในประเทศเลยที่เคยสูบบุหรี่ เราก็จะพบว่าในประเทศที่สองนั้น มีอัตราส่วนของผู้เป็นมะเร็งปอดที่ไม่เคยสูบบุหรี่เลยมากกว่า (เพราะไม่มีใครเคยสูบบุหรี่เลย)

จึงเป็นเรื่องปรกติที่ประเทศที่ฉีดวัคซีนมากกว่า จะพบผู้ติดเชื้อที่ได้รับวัคซีนครบแล้วเป็นสัดส่วนที่มากขึ้นเรื่อยๆ เนื่องจากว่าไม่มีวัคซีนใดที่สามารถป้องกันโรคได้ 100% แต่หากเราดูข้อมูลจริงๆ แล้วเราจะพบว่าอัตราผู้ติดเชื้อที่ได้รับวัคซีนครบแล้วนั้นมีน้อยกว่าสัดส่วนประชากรที่ได้รับวัคซีนแล้วเป็นอย่างมาก

ซึ่งนอกจากการป้องกันการแพร่เชื้อแล้ว อีกสิ่งหนึ่งที่วัคซีนทุกชนิดทำได้ดีก็คือ การลดอาการของโรคและอัตราการเสียชีวิต ซึ่งเราสามารถเปรียบเทียบได้โดยดู "doubling time of confirmed deaths" หรือ "ต้องใช้เวลากี่วันก่อนที่จำนวนผู้เสียชีวิตจะเพิ่มขึ้นเป็นสองเท่า" ซึ่งหากเราเปรียบเทียบกัน เราจะพบว่า

ไทย: 28 วัน
อังกฤษ: 216 วัน
สหรัฐอเมริกา: 216 วัน
ฝรั่งเศส: 222 วัน

ซึ่งจากข้อมูลที่มีอยู่ในตารางนั้น ปัจจุบันมีเพียงห้าประเทศ ที่มี death doubling rate เร็วกว่าประเทศไทย นั่นก็คือ เวียดนาม ฟิจิ อูกานดา กัมพูชา และนามิเบีย

ซึ่งหากเทียบสิ่งที่แตกต่างระหว่างประเทศที่ "ทำได้ค่อนข้างดี"​ และประเทศที่ "ยังต้องปรับปรุง" แล้ว ไม่ว่าจะด้วยเงื่อนไขใดก็ตาม สิ่งหนึ่งที่จะต้องพบอยู่เป็นประจำที่ไม่สามารถปฏิเสธได้ก็คือ "อัตราในการฉีดวัคซีน"

แม้ว่าเราจะทำได้ค่อนข้างดีในปีที่ผ่านมา เนื่องจากประชาชนให้ความร่วมมือ มีวินัยที่ดี แต่สิ่งที่ดีที่สุดที่เราจะทำได้ในยุคปัจจุบันที่มีวัคซีนป้องกันโรคนั้น ก็คือการหาวัคซีนที่มีประสิทธิภาพฉีดครอบคลุมให้กับประชาชนให้มากที่สุด ให้รวดเร็วที่สุด

เพราะในอีก 28 วัน จำนวนผู้เสียชีวิตที่ทะลุร้อยในวันนี้ จะเพิ่มขึ้นมาเป็นสองร้อย และสี่ร้อย ก่อนที่จะทะลุพันไปในที่สุด

Year 2020 was a challenging year for everyone including us as an Athlete.

We have proved it despite the Covid-19 Pandemic, we won't be having any mass event, but we were able to host a @guinnessworldrecords attempt. It cost us a total of RM 80k for this event, and we were able to secured it, special thanks to @tenaga_nasional and @allianzmalaysia for doubling up the sponsorship amount in the end.

Other than breaking the Guinness World Records, I would like to share a great news with all of you that since the end of 2019 from Guang Zhou International Vertical Marathon Final until my last race in Loros Tower Run, a total of 7 Towerrunning Races, I secured a 100% Winning Rate 🥇🥇🥇🥇🥇🥇🥇 (with 5 New Course Records).

Also after a 22 races between me and the World Champion @towerrunner Piotr Łobodziński since Aug 2017 in Beijing, I managed to beat him for the first time in my life. He is one of the Towerrunner that I looked up to since the first race in Beijing. He is my European Brother and we always went out to have meal, run here and there prior to the race and train together after the race. Looking forward to visit you in Poland and take part in the Warsaw Rondo 1 Tower Run.

In year 2021, it won't be easy and it filled with lots of uncertainties. My No. 1goal is to achieve World No. 1 in the sport when all the races are back and at the same time maintain all the winning streak!

Other than that, while there might not have any other events, I want to fine tune my performance and have a breakthrough for it to make the impossible possible!

Allen Iverson had every reason to feel good after his performance kept Philadelphia close to the division lead.

Surely the mood changed when the Sixers All-Star found out he was injured.

Iverson outperformed Kobe Bryant, scoring 36 points and leading a decisive third-quarter run in the 76ers' 108-91 win over the Los Angeles Lakers on Tuesday night.

Iverson helped pull the Sixers (31-33) to four games of Boston for first place in the Atlantic Division.

"It made me feel good," Iverson said. "Any win makes me feel good."

Iverson, though, chipped a bone in his left thumb sometime in the second half and will be fitted for a cast. He is day-to-day. The Sixers said Iverson will be re-evaluated Friday when they play again at Cleveland.

Iverson left the arena before the injury was announced.

The Sixers certainly need the league's leading scorer if they're going to make any kind of run at the Celtics.

"We would like to have that (winning) streak right now," said Iverson, before he knew the X-ray results.

Andre Iguodala scored 15 points for the Sixers, adding a few more spectacular dunks, including an alley-oop where he came crashing down on Jumaine Jones. Kyle Korver had 14.

Bryant -- who came in trailing Iverson by nearly two points in the league scoring race -- led the Lakers with 20 points, but had little help. The Lakers (32-31) fell out of the eighth and final spot in the Western Conference.

Bryant made five of nine shots in the first quarter, but went 1-for-6 the rest of the game.

"We've got to get more people involved," Bryant said. "They were doubling me and the shots weren't falling."

Iguodala had plenty to do with that. The Sixers rookie kept a hand in Bryant's face most of the game and aggressively contested his shots. Iguodala said he watched plenty of tape to prepare for one of his toughest assignments.

"I knew if I kept my hands up and kept nagging him a little bit that I could make him take a tougher shot," Iguodala said.

Bryant scored 16 points in the first quarter and Iverson had 14 as the two seemed poised for a one-on-one showdown between All-Stars. But Bryant slowed down, thanks largely to the sticky defense by Iguodala, while Iverson kept driving and crashing.

Iverson scored 14 points during a 26-9 run in the third quarter by doing what he does best -- drive the lane, look for the layup and get to the line. When he couldn't do that, Iverson would dribble around the perimeter until he found an open look.

He took six of Philadelphia's 16 shots in the third (making three) and went 7-for-7 from the line. Overall, Iverson went 14-for-15 from the line and 10-for-22 from the floor.

On one fallaway layup from the right side, Iverson was fouled by Chucky Atkins. Atkins bitterly complained and was hit with two quick technicals and ejected from the game.

Korver sank the two free throws for a 66-50 lead. Rodney Rogers hit a 3-pointer near the end of the third, giving Philadelphia a 28-point lead.

Chris Webber again was a non-factor for the Sixers. He scored Philly's first two baskets of the game, sat out 5 minutes of the second quarter and missed his last six shots for five points.

Webber remained baffled as to why he has looked little like the five-time All-Star he was in Sacramento. Of course, watching Iverson take a bulk of the shots and not having coach Jim O'Brien call plays for Webber might have something do with his struggles.

"I don't want to put my finger on it," Webber said. "It's not because it's late in the season. I didn't become this player in one day."

Game notes

Bryant, who went to nearby Lower Merion High School, needed about 30 tickets. Among those invited were players on this year's Merion team and his former coach, Gregg Downer. Lower Merion plays in the Class AAAA state semifinals Wednesday night. Bryant said he loved playing in Philadelphia, even though he's always booed. "If I played here, they'd love it with the Philly attitude I have," he said. "But I enjoy coming here because of that." ... The Sixers are 17-10 when Iverson scores 30 or more points

The last time the Philadelphia 76ers won 11 straight road games, Allen Iverson was 5 years old.

Iverson scored 30 points and pushed a frenetic pace as the 76ers tied a 21-year-old franchise record for consecutive road wins with a 114-98 victory over the Dallas Mavericks.

Philadelphia has not lost on the road since a 96-85 setback to the defending NBA champion Los Angeles Lakers on December 5. Its 18-4 road mark is the league's best and represents more wins than nine teams' totals for the entire season.

"It's hard on the road," Iverson said. "You've got to concentrate a lot more on the road. It's the toughest environment to play in. So you have to be focused and I think we've been real focused on the road. We understand everything is not going to go our way. We understand that we've got to keep playing for 48 minutes."

The 76ers tied a club record set from October 15-November 26, 1980, when the stars were Julius Erving, Andrew Toney and Maurice Cheeks, now an assistant coach with the club. They can break the mark Wednesday in Houston, which has a losing record at home.

"Coming into here, you know they beat us in Philly, and we were kind of shorthanded, not having (Rodney) Buford and Tyrone (Hill) to go with our other injuries," Sixers coach Larry Brown said. "It's been that way all year. We just find ways to win games and have guys step up and make valuable contributions."

"It's pretty good," forward Toni Kukoc said. "I think when you can win 11 games in a row on the road it tells you how good of a team we are. If we can keep our home court a little safer than what we've been doing, it would be nice."

Toni Kukoc scored a season-high 25 points for Philadelphia as he repeatedly knocked down open perimeter jumpers. He started for the flu-ridden Tyrone Hill and made 4-of-6 3-pointers.

"I was looking to pass the ball more than I was looking for my shot, then later on, I felt pretty comfortable shooting the ball," Kukoc said. "I had to find the good places and get open, especially when they started doubling. I spaced the court and my teammates found me and I hit the shots."

George Lynch added 16 points and 13 rebounds for the Sixers, who had six players in double figures and shot 54 percent from the field as they avenged a 112-94 home loss to Dallas on December 15.

In that game, Mavericks forward Dirk Nowitzki scored 36 points on 13-of-19 shooting. But the German sensation was held to 15 on 4-of-12 this time as Dallas had a three-game winning streak snapped.

"We just defended much better than we did in Philly but it didn't matter, I mean they were on fire in Philly," Brown said. "It wasn't just Dirk. I thought everybody on their team had a terrific game and made open shots. Today we stayed at home a little bit more. We didn't gamble quite as much or look to double-team as much."

"I didn't really get any open looks today," Nowitzki said. "Somebody was always standing by me. They did a great job defensively and you've got to give them credit."

Michael Finley had 27 points and a franchise-record 10 steals for the Mavs, who have lost five of the last six meetings with the Sixers.

"Gary (Mavs forward Gary Trent) talked to a couple of the Sixers yesterday that he's good friends with," Finley said. "That's all they kept saying was how we embarrassed them in Philadelphia. That was on their minds and it showed. They didn't give up. They came out and defended us."

Coming off a 51-point game against Toronto on Sunday, Iverson came out flying and never slowed down. He made 12-of-16 free throws and five steals as he cheated the passing lanes on defense and went hard to the basket on offense, forcing the tempo.

"He's one of the best players in the league," Finley said. "The way he handles the ball and as many times as he has the ball, he's always a threat. He's been playing well. He's been carrying this team."

Iverson scored 17 points in the first half, when Philadelphia led by as many as 13 before settling for a 51-48 halftime edge. Dallas tied it, 57-57, on a jumper by Steve Nash with 9:10 to go in the third quarter before the Sixers took control.

Iverson fed Lynch for a layup and made a pair of buckets. A 3-pointer by Kukoc and a jumper by Aaron McKie completed the 11-0 surge and gave Philadelphia a 68-57 lead with 6:32 left.

The Mavs got no closer than six points thereafter. They got within 77-71 late in the period but Kukoc made a 3-pointer. They closed to 71-75 early in the fourth quarter but a layup by Ratliff and a jumper by Kukoc rebuilt the lead to 10 points.

And when a free throw by former Sixer Shawn Bradley made it 85-79 with 8:41 remaining, Philadelphia sealed it with a 17-5 surge. Kukoc had another 3-pointer and capped it with a jumper for a 102-84 lead with 3:13 to go.

"Kukoc played well tonight," Finley said. "Toni was definitely the X factor that put them over the hump. If he plays like that, that team will easily be in the Eastern Conference finals."
